In order to enter a
dog in the any of our Field Tests,
you should complete the entry form below and
press the Submit Form button. You will
then be taken to our payment page. This
form must be completed by the owner of the dog,
or the trainer currently responsible for
the care of the dog. All documents and
certificates will normally be sent to the owner.
If the documents are to be sent to the trainer,
please indicate in the box at the foot of the
form. Part A is for the owner's
details - you do not need to fill in
your address if you are a registered handler.
If you are the trainer of the dog and not the
owner, please fill in your details in the
boxes provided. Part B is for the
dog's details. Part C is for the details
of the person who will be handling the dog in
the test if this is not the owner.

group discount, you may still use this
form. Each member of the group must fill in an
entry form. Ignore the payment page
